Our Approach
Resolute Care values partnerships including with the county case manager, the individual’s family, and other supports to best meet the person’s needs. We aim to be flexible and innovative, specifically when exploring new services and supports to offer an individualized program. Staff incorporate a person-centered, and strength-based approach when assessing the individuals needs.

Resolute Care, LLC is a Minnesota-based provider of high-quality Home and Community Based Services (HCBS) for individuals with disabilities and those aged 18 and above. Resolute Care believes that everyone should have the chance to live their best life, regardless of any obstacles they may face, and we are committed to helping them achieve that.
Our Community Residential Settings are licensed under 245D MN Statutes and are designed to feel like home, located in safe residential neighborhoods in small towns and rural areas. We offer a wide range of services for adults aged 18 and above with various developmental and intellectual disabilities, as well as mental illnesses, substance use disorders and co-occurring disorders.
